The initial prototype teeth did not fit, were off-center, and did not align with the screws, requiring two new sets within days.
The final prototype teeth remain too large and bulky, drastically changing my face shape. I voiced these concerns from day one, but my requests for modifications were repeatedly denied. I was initially promised remakes, then told a smaller set would cause implant failure.
After a three-month healing period, the clinic owner refused to provide smaller prototypes unless I paid extra. They also claimed a smaller zirconia set would break, demanding I sign a liability waiver if they altered the size. I do not think they are lying completely but I also have not been given other options to solve the issue for both sides. I’ve done a lot of research and have found yes the zirconia material is strong but can break easily if measurements or size are too small. I also have learned that this shouldn’t be the case if the maker of the teeth are very skilled and the scans they took of my mouth, gum, and face were done correctly.

Because the current prototypes do not look or feel right, I refuse to sign off on the final zirconia teeth. The office is uncooperative despite my full payment and consistent complaints.

– Penelope from San Diego

(See Dr. Hall’s answer below.)

We thank our advertisers who help fund this site.

<ins class="adsbygoogle"
     style="display:block"
     data-ad-client="ca-pub-4419877247826466"
     data-ad-slot="6484616093"
     data-ad-format="auto"
     data-full-width-responsive="true"></ins>

Penelope,
It sounds to me, from your description of your situation, that your problem isn’t with the crowns but with the placement of your implants. If your implants are positioned too far forward in your mouth, it will make the crowns that are placed on them look too bulky.

I’m assuming that when you had natural teeth there they looked fine and weren’t bulky. It shouldn’t be that difficult to replace those teeth with implant crowns that look the way your original teeth looked. But if the implant fixtures were put in your jaw in a position that is further forward than the original tooth roots, or are angled too far forward, the crowns placed on them will be too bulky. They can be made to look less bulky by thinning out the zirconia, but yes, you need a certain thickness of the zirconia or there is a risk of it breaking.

But then we get to the sticky part of your situation—how to fix it. It would be nice if the implants could be re-positioned. But that is complicated, will take a long time, and carries some risk. It isn’t easy to remove implant fixtures that have already fused to the bone. And then after they are removed, new bone would have to be grafted into the holes. After several months, when that heals, then new implants could be placed. After those implants fuse to the bone, you could have the crowns made.

It may be possible to give you an acceptable result without replacing the implants. There are two parts to an implant—the fixture that goes into the bone and the abutment that is placed on that fixture, on which the crown is placed. Maybe it could be possible with a custom-made abutment to position that abutment in such a place that the teeth would look like they are supposed to. A custom abutment costs more than the stock abutment shown in the diagram, but if it solved your problem it would be worth it.

You’re in San Diego. My advice would be to find an accredited cosmetic dentist in San Diego and get a second opinion from him or her to see if this is possible.

Good luck,
Dr. Hall

Do you have a comment or anything else to add?
